Owner as indicated above, having elected to purchase the extended service agreement for the period indicated above from CTVCam. This agreement goes into effect immediately upon documentation and  payment completion, when payment is received and or maintained for the period selected above. This service Plan covers maintenance of Security camera system hardware and cabling repair . 

Except for Hardware replacement  ie video cameras, camera accessories, connectors and cabling, CTVCam  agrees to service and repair the video camera system at no additional cost. To the Owner, each maintenance period or visit will be perform with-in 45 minutes (45 min.) of arrival to service location, to be completed with in 2.0 hours (2.0 hours) any additional labor time will be charged at full labor time or standard hourly rate,  based on  $65.00 hourly rate

The service maintenance agreement cover, 2.0 hour(2.0) free service call to customer location per month, ( the customer has to initiate the service call, ie call the service provider)  to be extended free of charge if the previous visit does not resolve the issue. 

The free on line service call or customer assistance will be scheduled and will not exceed 45 (forty-five)  minutes within a service period, not to exceed eight calls per month. Any additional service call or assistance will incur a service call fee of $35.00 and an hour

In addition the Customer / end user will receive a free quarterly maintenance service equipment service check, with in 90 miles of service provider / visit, to clean service, upgrade, review system file operation, and storage capacity,  end user will be informed of their system operational condition.

This service is required in order to maintain system operational stability and integrity it will also extend the lifetime of your Video Camera Surveillance system up to term ( from 5-10 years)  in addition you will receive a 10 % (ten percent) percent discount on any new purchase or upgrade or installation of new equipment or product.

This service agreement shall be invalid if the video camera system is tampered with or damaged from misuse, neglect, destruction, or alteration, improper use of electrical voltage or currents. In no event will CTVCam be liable for any consequential damages. This will be considered breech of agreement. / contract

The term of the Extended Service Agreement shall cover the entire period as selected by the Purchaser / end user beginning from the date of purchase, as shown on the Purchaser sales contract. CTVCam reserve the right to cancel this agreement for any alteration mis-use or abuse of equipment installed, and if payment are not received as scheduled. All legal rights, remedies, and the limitation thereof are reserved and acknowledge.
